If no, I understand and agree to participate in the *Discover New Life - Growth Track class given by New Life Church. The purpose of this class is to enable me to become a member of New Life Church and to therefore show that I take responsibility for the children that I will be ministering to. I agree to take this class the next time available.
The information contained in this application is correct to the best of my knowledge. I authorize any references of churches listed in this application to give New Life any information (including opinions) that they may have regarding my character and fitness for children's ministry. In consideration of the reciept and evaluation of this application by New Life Church, I herby release any individual, church, or organiztion both collectively and individually, from any and all liability for damages of whatever kind which may at any time result to me or my family on account of compliance or any attempts to comply with this authorization. I waive any right that I may have to inspect any information provided about me by any person or organization identified by me in this application. Should my application be accepted, I agree to abide by the by-laws and policies of New Life Church and to refrain from unscriptural conduct in the performance of my services on behalf of the church. I further state that I have carefully read the foregoing release and know the contents therof and I sign this release as my own free act. This is legally binding agreement, which I have read and understand.
